1. Discharge Patient

2. Preparation for Discharge
Discharge Summary: A discharge summary will be prepared, detailing your hospital stay, diagnosis, treatments received, and instructions for follow-up care.
Medication Instructions: The medical team will provide you with prescriptions for any medications you need to take after leaving the hospital, along with instructions on how and when to take them.
Follow-Up Appointments: Schedule any necessary follow-up appointments with your healthcare provider or specialists.
3. Patient Instructions
Care Instructions: Review and follow the discharge instructions provided by your healthcare team. This includes guidance on wound care, activity restrictions, diet, and medication management.
Emergency Contact: Know whom to contact if you experience any complications or have questions after discharge.
Follow-Up Care: Attend any scheduled follow-up appointments to monitor your recovery and address any ongoing medical needs.
Patient Feedback: You can provide feedback on your hospital experience through a feedback form or online portal, which helps us improve our services.
