Admission

Doctor's Recommendation: If your doctor recommends hospitalization, they will provide a referral or admission note outlining the need for inpatient care.

Admission Formalities

Location: MRD Desk

Admission Form: Complete an admission form with personal details, medical history, and insurance information. This form may be available online or at the hospital.

Documentation: Provide necessary documents, including identification, health insurance details (if applicable), and any previous medical records or referral documents.

Bed Assignment: Based on the availability a suitable bed in  Ward will be allocated to you.

Initial Assessment: Upon admission to your assigned ward/bed, a nurse or medical team member will conduct an initial assessment, including taking vital signs and reviewing your medical history.

Consent Forms: You will be asked to sign consent forms for treatment, procedures, and sharing of medical information as necessary.

Orientation: Staff will provide an orientation to your bed, including information about hospital policies, visiting hours, and contact information for medical and support staff.

Care Plan: A detailed care plan will be developed by your medical team, including scheduled treatments, medications, and any required diagnostic tests.

Monitoring: Regular monitoring and care will be provided by nurses and doctors throughout your stay. You will be informed of any changes to your treatment plan.

Instructions

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
